Andrea Leopardi 6a8305e719 Fix some grammar in the docs for Access, and add more docs 
- Added some missing words (à-la-typo) to the Access docs
- Added more detailed docs for `Access.get_and_update/3`
- Changed an empty start to a 1/2 Unicode char in the example because, well, it
  makes more sense :D

Andrea Leopardi da50690c6b Add an example of defining a __using__/1 macro 
The behaviour was explained pretty clearly in the docs, but for example it was
unclear that `__using__/1` has to be a macro. I added an example so that it's
easier to understand how to use `use/1-2` and `__using__/2`.

Andrea Leopardi bb9a9b16de Optimize the performance of Map.get_and_update/3 and Keyword.get_and_update/3 
I just shamelessly copied the implementation of `Access.get_and_update/3` for
`Map` and `Keyword` in order to optimize the performance of this function for
these two modules.

Andrea Leopardi 45d81bdc4c Add Dict.get_and_update/3 
I added the `Dict.get_and_update/3` function to the `Dict` behaviour (with a
default implementation like most of the functions in the `Dict` module); this
feature was discussed in #2973.

I also added test for the new function, docs with doctests and specs.

Martin Schürrer afa8d1522e Call Logger.flush in System.at_exit when running mix test. 
Otherwise messages logged by tests might not show up in the console
which makes finding out why tests failed a lot harder.
