Deprecate Enum.first/1 in favor of Enum.at/2 and List.first/1
...
The main reason for this change is because `Enum.first/1` does
not communicate that enumeration will happen, in contrast to
functions like Enum.at/2 and Enum.fetch/2 which are meant to
be used with non-indexed collections and the need for enumeration
is made a bit more obvious.
Closes #1983

Promote exception/1 as the best mechanism to customize exceptions
...
The previous mechanism of lazily customizing message/1 was very
dangerous, as you could have errors while calculating the message
and those would shop up just when the exception is printed, leading
to further breakage.

Require :escript_main_module for generating escripts
...
escripts must be opt-in because developers should not assume
they can simply generate escripts for all projects. This change
is a small one but makes it clear the desire for the project to
be escriptized.
