Provide faster record dispatching for protocols that do not have fallbacks.

Currently, if a protocol must be implemented for records but
not for tuples or any other data type, we simply dispatch to
the given tuple, assuming it is valid.
The downside is that we don't show `Protocol.UndefinedError`
messages if the protocol is invalid, but that can easily be
inflected from the underlying UndefinedFunctionError message.

Improve how exceptions are handled in general

* raise expects an explicit stacktrace in order to re-raise
* the Exception module now holds function for both stacktrace
manipulation and interface with erlang exceptions
* exceptions have their default functions with doc set to false

Update octal and hex escapes for characters, char lists, and strings to all accept
...
the same syntax and be case insensitive. Also relocated the character type macros
into lib/elixir/include/elixir.hrl so that they didn't need to be defined in both
lib/elixir/src/elixir_interpolation.erl and lib/elixir/src/elixir_tokenizer.erl.
Added to the erlang and elixir unit tests to test the new syntax.
