Raw function definition now evaluates all arguments
...
This applies to def/4, defp/4, defmacro/4, defmacrop/4.
The previous behaviour was accidental and did not properly
evaluate all arguments.

ExUnit now supports after_spawn callbacks
...
Such callbacks are invoked after each process is spawned.
This commit also refactors ExUnit.Runner slightly to consume
information from ExUnit.Server gradually instead of retrieving
it all up-front. This small step allows us to have ExUnit
running in the background in the future or to run many suites
at once.

Remove extended semantics from the access protocol
...
The semantics of the access protocol is now reduced to a
key based access in data structures. This suits better
Erlang's philosophy of not overloading operators with
different semantics.
